Home health agencies in New Hampshire, as a condition of licensing, are required to furnish evidence of a surety bond.
The amount of surety bond required is:
Qualifying for the $50,000 New Hampshire home health agency medicaid bond is just by completing an application --- which our surety bond agent will take from you over the phone --- and payment of the premium, 2%, or $1000.00. There is no credit check nor underwriter review for the $50,000 surety bond amount.
The application consists of the basics --- your company name, address, and other contact information. We’ll also ask you for your Provider Number, which number will be reflected on your completed bond.
Finally, we’ll need you to provide us with the name of the entity who is requiring your surety bond and their address, and the state this entity is located in.
For any amount greater than $50,000, our agency will need the same surety bond application from you; however, you will also need to sign and complete the application and return it to us by email.
Thereafter, your application is submitted to an underwriter for review. Once approved --- and good credit is required --- the premium will be at the same rate of 2% of the surety bond amount.
After approval by our underwriter and payment of your premium, our agency issues HHA surety bond to you immediately. Your bond and an accompanying Power of Attorney (POA)form will be mailed to you the same day. Upon receipt, you’ll sign your bond as Principal and file it along with your POA.
Your home health agency medicaid bond is active for one year from the date of issue. Your bond will renew on the anniversary of its issuance, and if a new surety bond amount is required, we won’t need an entirely new application.
The bond is required to ensure your compliance with the Secretary of the Department of Health and Human Services, in accordance with the Balanced Budget Act of 1997.
